  • Electronic Addresses: Email Addresses, Web URLs, Internet Protocol (IP) Addresses.
  • Identification Numbers: Social Security Numbers, Medical Record Numbers, Health Plan Beneficiary Numbers, Account Numbers, Certificate/License Numbers.
  • Vehicle/Device Info: Vehicle Identifiers and Serial Numbers (including license plate numbers), Device Identifiers and Serial Numbers.
  • Biometrics & Images: Biometric Identifiers (including finger and voice prints), Full-face Photographic Images and Any Comparable Images.
  • Other: Any Other Unique Identifying Number, Characteristic, or Code that could be used to identify the individual, either directly or indirectly.

3. How We Use Information

Your information is used to provide services, notifications, improve quality & performance, share updates, and offer the best In-app experience. Services may include but are not limited to managing appointments, providing access to medical records including histories, prescriptions, lab reports, display doctor schedules, and handle billing information. We also use your data to improve app functionality and user experience and for any other purpose you give us authorization for.

We may use information that has been de-identified—meaning it no longer contains personally identifiable information—without restrictions for any purpose.

5. Sharing and Disclosure of Information

We may share your data with healthcare providers and authorized third parties (such as billing services, associated labs, pharmacies etc.) to facilitate the services offered by MED360. We ensure that all data-sharing practices are compliant with International standards, and we do not sell or share personal information for commercial purposes.

We adhere to International standards for healthcare data exchange and regional compliance to ensure the highest level of data security, and all of your data is highly encrypted at all times even where data exchange may be necessary with authorized 3rd parties and associated partners.
